- Proposed Project Dates and Cost - Anticipated Start and End date (can be in the past if project is complete)
- Proposed Project Total Cost - the cost of the project not considering any particular source of funding
- Grant Amount Requested - There is 5% cap ($2,487,500) per recipient. Please do not request more than we are statutorily able to award

Collect Electronic Copies of Documents You Will Need to Upload into Application
- Damage documentation (reports, images, etc.)
- Proposed Project – Scope of work including breakdown of costs by category and a detailed project budget based on grant amount requested
- FEMA denial documentation (email, letter, or otherwise, please include a typed description if you provide a supplemental document from a formal letter)
